CITY OF SAN FERNANDO, Pampanga — The city government of San Fernando continues to accept nominations for The Outstanding Fernandino Awards (TOFA) 2019.
In a statement, Mayor Edwin Santiago said the annual accolade aims to recognize Fernandinos who have excelled in various areas and have significantly contributed to the enhancement of the honor and reputation of the Fernandinos.
“Fernandinos may be nominated in a wide range of categories including religious service, education, environment care and preservation, law, agriculture, science and technology, social service, medicine, military and law enforcement, business, culture and arts, community service, parenthood and family, senior citizens service, sports, media, youth service, government service, entrepreneurship, and person with disability,” Santiago explained.
Nominees must either been born in the city, or at least one of his or her parents is a Fernandino, or must be a resident of San Fernando for the past 15 years; and must be of good moral character.
“The nominees must also have outstanding achievement in their chosen field with corresponding proof or documentation, and must be nominated by an individual, group of individuals or recognized organization in San Fernando,” the local chief executive added.
Nomination forms are available at the City Tourism Office or may be downloaded in www.cityofsanfernando.gov.ph.
Nomination forms, together with the nominee’s birth certificate, 4R colored picture in business attire and other supporting documents may be submitted to the City Tourism Office or thru e-mail at [email protected].
